The LED error is fatal, so you should approach your supplier for a warranty replacement.

All Pis are tested before leaving the factory, so this fault has developed post test. It's rare but does happen. When you are making 90k devices a week, even a very small failure percentage is a few devices. This is probably a soldering failure, worked during test, but transportation worked something lose. With regard to packaging, we are happy with the current system, as long as the board is correctly manufactured It works fine.
The packaging system is poor. I’ve never known printed circuit boards to be packaged naked in a cardboard box before and I fit pcbs all the time. They usually come in an anti static bag with foam protection inside the box
You should probably meet this guy, who is also an expert on why the eleventy-billion Pis already shipped were packaged poorly:

If you fit the same PCB all the time in your job you're never going to see a PCB packaged any other way. It's a single example that you really shouldn't extrapolate to all PCBs ever made. The cause of your specific problem is unknown, so you can't say it's the packaging, therefore you can't say the packaging is poor, nor can you say what specifically would improve it.
I don’t fit one pcb, I fit many different pcbs from many different manufacturers on many different boilers. Never before have I come across a pcb naked in a cardboard box without some sort of protection. The fact that mine and others have arrived damaged proves packaging is needed. If it’s a cost issue then increase the price to suit.
